BACKGROUND:
The City Council awarded RFP 22-071, EMS & Non-EMS Billing Services, on May 17, 2022 to Ambulance Medical Billing (AMB) for a three-year term with two, one-year options to extend. The purpose of this contract is to provide EMS and non-EMS billing services to support Fire Department billing and collection services.
Per the Naperville Municipal Code Section 1-8B-4, the Fire Department is responsible for maintaining and providing ambulance (EMS) and other emergency services (non-EMS).
The Code also establishes the billable amounts which are charged to residents and non-residents for Fire Department responses.
DISCUSSION:
AMB has met the requirements of the contract during the original term and staff recommends extending the contract for the first option year.
The Fire Department will begin a pilot program to evaluate in-house billing for EMS and non-EMS during the fourth quarter of 2025. If successful, these services could be administered by City staff, thus reducing or eliminating the need for third-party billing services.
The term of the contract is from June 30, 2025, through July 1, 2026, with one, one-year option remaining.

EMS and non-EMS billing services are expensed to the account listed below. A total of $336,000 has been budgeted for 2025 and $206,000 for 2026, which reflects the potential reduction in fees to be realized through in-house billing. Fees are assessed as a percentage of the amount billed and are offset by the revenues collected. Estimated revenues are $9.2 million in 2025 and $9.55 million in 2026 for Ambulance Fees.
